It’s important to file a workers’ compensation claim as soon as possible after the injury. Why? The sooner the claim is filed, the sooner medical treatment and other services can begin to help get the injured worker back on the job. And that helps keep employers costs low.

Filing the First Report of Injury, Occupational Disease or Death (FROI) form is quick and easy. Click here to complete the FROI online and submit it directly to the Ohio Bureau of Workers’ Compensation (BWC).

BWC will assign a claim number and 1-888-OHIOCOMP will notify the employer’s assigned case manager to coordinate medical treatment and a safe, early return to work.

Or you can print the FROI from the above site, complete and mail it to 1-888-OHIOCOMP, 2900 Carnegie Ave., Cleveland OH 44115; or fax it to 1-888-644-7339.
